Day Care Enrollment Form

This form is required for all students.

Non-Medication Consent Form

This form is for over the counter topicals such as sun screen. We are a non-MAT program and so cannot administer medicine.

Parental Consent Form

This form is required by all students who will have outside parties come in for them such as therapists.

Child in Care Medical Statement

This form is required by all students. It involves a Doctor.

Individual Allergy and Anaphylaxis Emergency Plan

This form is required by all students with an allergy. It involves a doctor.

Napping Agreement (Only Full-Time)

This form is required for all students.

Individual Health Care Plan for a Child with Special Health Care Needs

This form is required by all students with special health care needs.
